Primary and Secondary IV Infusions: A Practical Guide for Healthcare Professionals

Intravenous piggyback (IVPB) administration is a commonly used method of delivering intermittent medications through an established intravenous infusion system. While IVPB therapy is a routine clinical procedure, safe administration requires knowledge of primary and secondary infusion systems, medication compatibility, aseptic technique, infusion pump programming, and appropriate patient monitoring. Improper setup or administration can result in delayed or incomplete medication delivery, infusion errors, and potentially serious patient complications.

This two-contact-hour continuing education course provides healthcare professionals with a focused review of evidence-based IV piggyback administration. Based on the 2024 Infusion Nurses Society (INS) Infusion Therapy Standards of Practice, current medication safety recommendations, and relevant peer-reviewed literature, the course examines the principles of secondary infusion therapy, appropriate equipment selection, medication verification, tubing preparation, back-priming, infusion pump programming, and post-infusion care.
Special emphasis is placed on recognizing common IVPB administration errors, troubleshooting infusion system problems, verifying complete medication delivery, and preventing complications through ongoing patient assessment and safe infusion practices.

This course is designed for licensed nurses and other healthcare professionals who administer or assist with intravenous medications within their authorized scope of practice. Because IV medication administration responsibilities vary by profession, state regulations, and healthcare organization, participants must verify that IVPB administration is permitted within their individual scope of practice and facility policies. Completion of this course provides continuing education but does not independently authorize IVPB administration or replace required hands-on competency validation.

Objectives

Upon completion of this course, the participant will be able to:

  1. Differentiate between primary continuous infusions and secondary intermittent (IV piggyback) infusions.
  2. Identify appropriate equipment, medication compatibility requirements, and aseptic practices for IVPB administration.
  3. Describe the correct preparation, setup, and programming of a secondary infusion using an electronic infusion device.
  4. Recognize common IVPB administration errors and implement appropriate troubleshooting and prevention strategies.
  5. Explain patient assessment, post-infusion care, and documentation requirements for safe IVPB medication administration.
