Chronic diseases are among the most prevalent and costly health conditions worldwide and are on the rise. Chronic diseases are the leading cause of death and disability. Chronic disease ranks poorly on cost and outcomes, primarily because of the ineffective management of the chronic disease. Nurses will play an integral role in the future prevention, treatment, and control of chronic disease, within a collaborative care model, to achieve improved clinical outcomes and more appropriate use of health services. The development of nursing led programs, to provide preventative education and encourage self-management, by chronically ill patients, will help achieve the global strategic goals of building healthy community environments, making more quality preventative services available, educating people so they can make healthy choices, eliminating healthcare disparities and ultimately decreasing the heavy global burden of chronic disease.
