Medication Administration and Best Practices to Deprescribing - Online Inservice

This course is designed to provide healthcare professionals with the knowledge and skills necessary to implement effective deprescribing practices in long-term care (LTC) settings. Deprescribing, the process of tapering, stopping, or withdrawing medications that may no longer be necessary or may cause harm, is a critical intervention for improving the health and quality of life of older adults who often take multiple medications. Participants will explore the principles of safe medication administration and how to apply best practices for identifying when deprescribing is appropriate. The course will cover key strategies for reducing polypharmacy, managing medication regimens, and collaborating with interdisciplinary teams to ensure the safe and effective withdrawal of medications. Learners will understand the importance of individualized care plans that prioritize patient safety and well-being during deprescribing efforts.

Objectives

Upon completion of this course, participants will be able to:

  1. Identify the key principles of safe medication administration and their role in ensuring patient safety during the deprescribing process in long-term care settings.
  2. Explain the deprescribing process, including when and how to taper, stop, or withdraw medications that are no longer beneficial or may be causing harm to residents.
  3. Evaluate the risks and benefits of deprescribing for residents taking multiple medications, and develop strategies to manage polypharmacy safely and effectively.
  4. Collaborate with interdisciplinary care teams to assess medication regimens, determine the need for deprescribing, and create individualized care plans for residents.
  5. Implement best practices for monitoring and follow-up during the deprescribing process to ensure resident safety, address potential withdrawal symptoms, and adjust care plans as needed.

She is a Fellow in the American College of Health Care Executives, a Licensed Nursing Home Administrator, and a Registered Nurse.
